Incident Narrative

An employee was removing a roll shaft from an expired roll of paper. The employee put the other end of the roll shaft into the new roll to use the weight to free the old core. When the core came free, the shaft going into the roll pinched the employee's finger between the core and the roll shaft, resulting in amputation of the right middle fingertip.
