Incident Narrative

An employee was jogging a cylinder cart onto the lift gate of a service truck when he lost his balance and his right leg was caught by the cart, leaving him hanging upside down. He managed to pull himself back up and fell from the lift gate (3.5 to 4 feet high) to the ground below, fracturing his left tibia.
