Incident Narrative

An employee leaned half of an extension ladder against the side of a power unit and started to climb up while two other employees held the sides of the ladder. When he extended his hand to receive a nozzle from one of the other employees, the ladder slipped out from underneath him. He fell approximately 4 to 5 feet and hit the ground, breaking his ankle.
