Incident Narrative

On March 1, 2018, an employee was repairing a tank engine and revving it manually near an exposed turbo air inlet fan blade. The employee's right hand then came into contact with the rotating fan blade, amputating the index and middle fingers. The air intake was unguarded at the time of the incident.
