Incident Narrative

An employee was attempting to relocate a 10-inch flange by hand. During the relocation, the flange started to roll onto its side and the employee attempted to control the flange catching the employee's right middle finger between the rolling flange and another flange on the pallet resulting in a fingertip amputation.
