Incident Narrative
An employee chipped concrete from inside a mixer. After re-energizing the mixer, the employee noticed that the chipping tool attached to the airline was still inside the mixer. When the employee attempted to retrieve the tool, the mixer paddle cut the employee and caused muscular damage. The mixer not locked/tagged out at the time.
