Incident Narrative

A welder employee was stamping a part number onto the metal rim of a tire with a metal stamp and carbon steel hammer. When the employee struck the metal stamp with the hammer, the hammer shattered and shrapnel struck the employee in the midsection. The employee was hospitalized for surgery to remove the metal pieces.
