Incident Narrative

An employee helped a driver reconnect a dumpster to the dumping mechanism on the back of a truck. The driver then went to the other side of the truck to operate the dumpster, but the bin slipped out again. The employee again went to re-attach the dumpster as the driver began to operate it. The employee's right middle fingertip was caught between the dumpster and the dumping mechanism, amputating it.
