Incident Narrative

On January 31, 2018, an employee was processing returned product (a 3-bar bundle of 6-inch diameter round 120-inch long steel bars). He was cutting the metal straps that secured the bundle to the pallet. When he cut the fourth and final strap, the top bar sank down between the two bottom bars and one of the bars rolled onto his foot, fracturing his first and second metatarsals. He was hospitalized.
