Incident Narrative
A temporary employee was driving a forklift to the battery charging area when he struck the bollard around a pole. The employee was not wearing a seat belt and struck the safety cage. When he tried to get off the lift, he passed out, fell, and hit his head on the concrete floor. He was hospitalized and had surgery for a broken neck.
