Incident Narrative

An employee was working at a height of about 15 feet to remove gang forms and strip out a bulk head with another employee working on the opposite side of him. The employee was wearing a positioning belt and was tied off to an anchor point affixed to a rebar. As he was working, the positioning belt's hook slipped off the tie-off point and he fell, striking a protruding rebar at a lower level and puncturing his right thigh.
