Incident Narrative
An employee was putting a large box of foam insulation on the top shelf of a storage rack. He was extended over the top rail of the warehouse platform ladder. As he was pushing on the box, the ladder tipped over, and he fell approximately 4 feet to the shop floor, fracturing two vertebrae.
