Incident Narrative
An employee was standing on one side of a rotary drilling bit while a forklift was on the opposite side, pushing the center piece out of a rotary drilling bit. The employee was holding a metal square plate to help remove the center piece when the center piece became dislodged from the rotary. The employee's hand was then caught between the metal square plate he was holding and the center piece. He sustained an amputation to his right index and middle fingers and a laceration to his left thumb.
