Incident Narrative

A temporary employee was using a plastic part to dislodge cheese pieces from a cheese slicing machine's running conveyor. The plastic part and the employee's left hand were pulled into the conveyor. The machine's blade then amputated her middle finger, lacerated her index finger, and injured her ring fingernail bed. She was hospitalized.
