Incident Narrative

An employee was using a metal shear to shear an 8' x 16" wide aluminum scrap roofing material to 4" x 4" test strips when his hand went under the guard and the stops came down and crushed both of his index fingertips. His left index finger required a 5 mm amputation from the tip end of the bone. His right index finger was debrided.
