Incident Narrative

An employee was helping two other associates close a door to a cardboard baler. The injured employee and a coworker were pushing on the door while another was operating the baler controls. The injured employee instructed a coworker to close the hydraulic clamp that closes the door. The hydraulic clamp crushed the employee's four left-hand fingers (index through pinky) between the door and hydraulic clamp. The fingers were amputated.
