Incident Narrative

An employee was cutting through a weld on a glass furnace using a grinder with a cutting wheel. The cutting wheel broke after getting stuck between two pieces of metal and the employee lost control of the grinder, which then scraped the employee's chest and stomach and lacerated the right leg above the knee. The employee was hospitalized.
