Incident Narrative

An employee was polishing a metal bar on a manual lathe with an emery cloth when the cloth became caught around the rotating bar, consecutively catching the employee's glove. The lathe was rotating at 566 rpm at the time of the incident. The employee's right index finger was amputated to the first knuckle.
