Incident Narrative

Two employees were replacing a 10-inch blow-down pipe that was approximately 20 feet long beneath a conveyor. They attached a rope to the pipe and pulled it along the conveyor using a forklift. While they were moving the pipe along the conveyor, it became caught on another piece of pipe. The forklift operator kept adding pressure to the rope. One of the employees went to free the pipe, and it shot forward due to the pressure from the forklift. The pipe struck the employee and pinned his right arm against the conveyor's leg, breaking it.
