Incident Narrative
A crew was setting an air release vault lid onto an air release vault when the load shifted. The employee's right-hand fingers were then pinched between the lid and the vault, breaking the ring finger and causing minor lacerations to the middle and index fingers. His pinky digit was later amputated.
