Incident Narrative
On December 6, 2017, at 7:30 a.m., an employee was unloading a lumber order at the front of a construction site with a truck-mounted forklift when he hit an irregularity in the ground surface. The forklift then pitched forward, slamming his forehead into the rollover protection structure cage and rocking him back into the seat. He received a forehead laceration and fractured vertebrae, requiring hospitalization and surgery. A seatbelt and hardhat were not worn at the time of the incident.
