Incident Narrative

On 12/5/2017, an employee was using a reciprocating saw to cut a pipe while standing on a 12-foot stepladder. While he was making a second incision in the pipe, it swung down, hit the ladder, and knocked it over. He fell and his face hit the concrete floor, breaking a facial bone and causing cranium bleeding. He was hospitalized and had surgery.
