Incident Narrative
An employee was helping to free a stuck pallet from the forks of a hand truck. She stepped up onto the pallet to use her weight to hold the pallet down. A second employee pulled on the hand truck and the momentum caused the employee to lose her balance and fall, injuring her hip.
