Incident Narrative
On November 27, 2017, at approximately 1:45 p.m., employees were replacing bad decking and roofing. Materials were torn off and handed to the injured employee, who then placed the materials in a cart and transported them to a chute. He was receiving materials when he tripped and fell into an area where the decking was bad. He fell 60 feet to the floor below, fracturing his left leg.
