Incident Narrative

An employee was on top of an air handler, troubleshooting it. As he came down, he twisted his ankle and fell on the cover of the air handler's pulley/belt drive, knocking the cover off of the rotating pulley system. Two of his fingers were then caught and amputated in the rotating pulley system.
