Incident Narrative

An employee was in an aerial lift using a chain fall to plumb a steel column and set a row of purlins when the lift's rise button was activated. His head was then caught between a purlin and the lift at a height of 13 feet. He suffered a laceration above his left eyebrow, a cut to his lower lip, and a hairline fracture to his cheekbone.
