Incident Narrative
An employee was repairing a metal platform that was supported by metal saw horses and jack stands. A metal shim was in place on the saw horse to level the platform. The employee noticed the shim had turned slightly, so he used a 4 lb. maul to tap the shim into its original position. The force of the strike caused the shim to dislodge, and the corner of the platform came down on the employee's index finger. The employee suffered an amputation.
