Incident Narrative
An employee went to the south side of a machine to grab an extruded plastic sheet and realized the roller assembly was in the up position instead of down. While the assembly was lowering, the employee leaned forward over the tote bin. The roller assembly continued and made contact with the employee's back, causing the employee's torso to be wedged between the roller assembly and tote bin resulting in a fractured sternum.
