Incident Narrative
An employee was removing a flange from a client's oil/gas well with a hydraulic torque wrench. While the employee was placing the torque wrench on the nut, the employee's finger was in a pinch point between the torque wrench and nut. The employee then inadvertently stepped on the controller, which activated the torque wrench, amputating the employee's right index fingertip and nail.
