Incident Narrative
An employee was preparing to stow an auger drive shaft. To prevent the drive shaft from dragging on the ground, the employee planned to have the shaft tucked under a boom and temporarily tied off in an elevated position. Overhead obstructions prevented the employee from raising the boom and moving forward. To get the drive shaft into position, the employee stood to the side of the boom and had just started to pull the drive shaft toward the back of the truck when the boom was lowered. The tip of the drive shaft then contacted the employee s foot, injuring the big and second toes and requiring surgery.
