Incident Narrative

An employee was prepping the stockroom to receive a distribution center merchandise truck. He was walking past a U-Boat with freight when he bumped into the U-Boat with his right arm, which caused the boxes to fall. The employee then tripped over the boxes and fell, fracturing his left hip.
