Incident Narrative
An employee was installing a ladder safety system onto the top of a ladder when the ladder davit fabrication slipped and hit his finger. His finger was caught in a pinch point between the davit fabrication and t-cage steel, amputating his fingertip and causing an open fracture to the finger (partway up the nail).
