Incident Narrative

A temporary employee was operating a machine and holding a clicker to count/advance the machine when the employee dropped the clicker onto a shelf. As the employee reached to retrieve the clicker, the employee's left hand was caught in the machine, amputating the middle fingertip and fracturing the ring and middle fingers.
