Incident Narrative

An employee was working at a stationary grinding machine to grind off the top layer of a conveyor belt. The employee was wiping debris off the top of the conveyor belt as it came out of the grinding wheel when the employee's right index finger came in contact with the grinding wheel, amputating the tip.
