Incident Narrative
An employee was working with a winch to pull a cable with steel ribbing to tighten the ribbing against lagging boards. He had a variable speed controller in one hand to operate the winch, while his left hand was on the cable. When the controller was activated, it pulled his hand into the winch, amputating his left index, middle, ring, and pinky fingers from the knuckle up.
