Incident Narrative

An employee was cutting a steel object with a propylene/oxygen cutting torch when the remaining steel piece (weighing 2,320 lbs.) shifted and started to fall. The employee tried to walk away, but tripped on an uneven surface and fell into the path of the falling steel piece. The steel contacted his upper thighs and the weight of the material dislocated and fractured his right ankle.
