Incident Narrative

An employee was drilling a hole into a fixture with a lathe on 9/21/2017. The employee leaned into the machine to line up the drill bit and the employee's shirt was caught in the worm-screw resulting in a left arm fracture and a stroke. The worm-screw was not guarded at the time.
