Incident Narrative

An employee was ascending a crank-up scaffold system via an affixed access ladder when one of the employee's hands slipped from a rung. The employee then fell approximately 3 feet to the finished concrete surface below, twisting and breaking the right ankle. The employee was hospitalized and had surgery.
