Incident Narrative
On September 11, 2017, at approximately 1:30 PM, two employees were performing service on a forklift (one employee was adjusting tension on the fan belt/the second was lubing the steering axle). The second employee attempted to start the forklift and the first employee's left hand was caught between the fan belt and pulley, amputating the middle, ring and pinky fingertips.
