Incident Narrative

An employee's cellphone fell onto a conveyor belt while he was removing a jammed stick from the belt. As he attempted to retrieve his phone, the conveyor caught his right arm, removing flesh from his bicep and scraping his arm from his elbow to his knuckles. He was hospitalized.
