Incident Narrative

An employee and a coworker placed an approximately 52-pound box on a shelf. The box shifted and slipped off the shelf, striking the employee on the shoulder after he had turned his back and was walking away. He fell to the ground and struck his head, suffering a concussion that required hospitalization.
