Incident Narrative

An employee was painting the walls and ceiling in a school room. While descending a 12-foot A-frame ladder, he felt dizzy, missed a step, and fell 8 or 9 feet to the floor, bruising his head, scraping his right arm, and fracturing his right hip. He was hospitalized.
