Incident Narrative

An employee was descending from a ladder into a railcar. He pulled his retractable lifeline out so when he hopped down the last step, the automatic stopping mechanism would not engage on his personal fall arrest system. When he jumped, the arrest system still engaged and pulled his body back, causing him to land on and twist his right foot. His right ankle broke and required surgery.
