Incident Narrative

An employee was checking the tension on a v-belt as part of preventative maintenance. When he grabbed the belt, it suddenly activated and pulled his hand into the pulley system, amputating his right ring and middle fingertips. The belt was unguarded and the conveyor was not locked/tagged out at the time.
