Incident Narrative

An employee was preparing an inflatable boat for training. He attached an air tank to the boat floor using quick-connect attachments. A rescue board was secured inside the boat. When he turned on the air tank, over-pressurization caused the floor of the boat to rupture and the rescue board to push up and shove him into the air. One of the hose attachments then separated from the floor and struck his chin. He suffered a compression fracture to his left ankle requiring surgery and a chin laceration requiring stitches.
