Incident Narrative
An employee was helping to remove forms from a parapet wall over the interstate. Tension was being placed on the form from the opposite side to assist in the removal. The form broke loose from the parapet wall, swung, and struck the scaffold guardrail and the employee. The guardrail broke, and the employee fell approximately 25 feet to the shoulder of the roadway, breaking a leg and suffering lumbar fractures.
