Incident Narrative

An employee was hoisting up the scale weight with the help of another employee. The injured employee had a hand on the cable that held the hoist. When the scale weight got near the floor, the employee's right hand was caught in the pulley and the index finger was smashed. The employee's index finger was amputated and the middle finger injured.
