Incident Narrative

An employee was mowing a lawn on a slope using a push mower when it slipped and went over the employee's right foot. The mower blades went through the employee's steel-toed boot and cut the middle toe on the employee's right foot. The tip of the employee's middle toe was amputated at the hospital.
