Incident Narrative

An employee was helping a coworker remove a 5/8" bolt on an inlet to service a valve that was 2" from the wall. The employee was checking the depth of the nut on the bolt when the coworker turned the nut with a wrench, pinching and severing the employee's right ring finger skin to the bone.
