Incident Narrative
An employee was replacing forklift cylinder seals on the outer main lift cylinder. The employee raised the mast channels to separate the cylinder from the mast. The jack failed and the employee's right hand was caught between the cylinder and the forklift mast resulting in a crushing injury to his right hand.
